The grading system I use is based on the 'Record Collectors Grading System' with the addition of the Excellent (EX) category.
A + or - after the grading gives more detail to the condition of the record, e.g. VG+ is in better condition than VG as detailed below.

MINT (M)

The record is absolutely perfect, in brand new condition with no surface marks or deterioration in sound quality. The cover and any extra items such as the lyric sheet, booklet or poster are in perfect condition. Records advertised as Sealed or Unplayed should be Mint.
NEAR MINT (NM)

The record looks brand new with no scuffing or any deterioration in sound quality and only the smallest hint that its ever been played. The cover and any additional extras are clean and crisp with no wear whatsoever.
EXCELLENT (EX)

The record shows some signs of having been played, some light surface marks may be evident, but there is very little lessening in sound quality. The cover and packaging might have slight wear and/or creasing.
VERY GOOD+ (VG+)
The record shows some signs of wear, it has surface marks and possibly some very light scratches that don't affect the listening experience. Slight warps that do not affect the sound are acceptable. The label may have some ring wear or discoloration. Sleeves will have some light wear, lightly turned up corners, or a slight seam split, and may be marred by a cut-out hole, indentation or corner.
VERY GOOD (VG)

The record has obviously been played many times, but displays no major deterioration in sound quality. Groove wear will start to be noticeable and some surface noise will be evident upon playing. There will be surface marks and light scratches (deep enough to feel with a finger) that will affect the sound. Labels may be marred by writing, or have tape or stickers (or their residue) attached. Sleeves wear is more noticeable than in a VG+ graded sleeve .
GOOD (G)

The record has been played so much that the sound quality has noticeably deteriorated, perhaps with some distortion and mild scratches. The cover and contents suffer from folding, scuffing of edges, spine splits, discolouration, etc.
CDs: the CD must play perfectly to be listed, the grading ranges from Mint (brand new) to Excellent (marked but plays fine) in condition.
